STEM TutorLab Services Agreement
Tutoring Service Provider: STEM TutorLab
This Tutoring Services Agreement (“Agreement”) is entered into on the date entered below between the Tutoring Provider and the student listed below.
1. Tutoring Services
The Tutor agrees to provide personalized STEM tutoring in the following subject or subjects:
- Mathematics
- Chemistry
- Biology
- Physics
- Engineering
- Computer Science
- Test Preparation

4. Payment Plan Terms
Students and/or Parents/Guardians who choose a payment plan agree to the following terms:
- The first payment is due in full before the first tutoring session. Tutoring services will not begin until the initial payment has been received.
- Each subsequent payment is due no later than the final scheduled tutoring session covered by the current payment period. Payment must be received before the next payment period begins.
- Failure to submit payment by the due date may result in the suspension of tutoring services until the outstanding balance is paid.
- If payment is not received before the first session of the next payment period, the Tutor reserves the right to cancel or reschedule upcoming sessions until the account is brought current.
- By selecting a payment plan, the Student and/or Parent/Guardian acknowledges responsibility for making all scheduled payments according to this Agreement, regardless of attendance or early termination, unless otherwise agreed to in writing by the Tutor.
5. Cancellation and Rescheduling Policy
The Student or Parent/Guardian must provide at least 48 hours' notice to cancel or reschedule a tutoring session.
Sessions canceled with less than 48 hours' notice may be charged at the full session rate and may not be eligible for a refund or rescheduling.
Missed sessions without prior notice ("no-shows") will be charged at the full session rate and will not be rescheduled.
For students enrolled in a monthly payment plan, any canceled session that qualifies for rescheduling must be rescheduled and completed within the same calendar month. Sessions that cannot be rescheduled within that month will be forfeited and will not carry over to the following month.
For students enrolled in a weekly payment plan, any canceled session that qualifies for rescheduling must be rescheduled and completed within the same week. Sessions that cannot be rescheduled within that week will be forfeited and will not carry over to the following week.
Rescheduled sessions are subject to the Tutor's availability.
If the Tutor must cancel a session, the session will be rescheduled at a mutually agreed-upon time or credited toward a future session at no additional cost to the Student.
6. Cancellation and Rescheduling Policy
The Tutor agrees to:
- Prepare lessons based on the student’s subject, grade level, and academic goals.
- Provide a professional, respectful, and supportive learning environment.
- Arrive on time and remain available for the agreed session length.
- Explain concepts, provide practice, and help the student strengthen problem-solving skills.
- Provide reasonable feedback regarding the student’s progress.
- Maintain appropriate professional boundaries with the student.
7. Student Responsibilities
The Student agrees to:
- Attend each tutoring session on time.
- Arrive prepared with textbooks, assignments, notes, calculators, and other required materials.
- Participate actively and make a reasonable effort during each session.
- Complete agreed practice assignments when applicable.
- Communicate areas of difficulty and ask questions when clarification is needed.
- Treat the Tutor respectfully and avoid disruptive behavior.
Tutoring support does not replace the Student’s responsibility to attend class, study independently, complete assignments, and follow school academic policies.
8. Parent or Guardian Responsibilities
When the Student is under 18, the Parent or Guardian agrees to:
- Ensure that the Student attends each scheduled tutoring session.
- Provide accurate contact, scheduling, and academic information.
- Provide a safe and appropriate environment for online or in-person tutoring.
- Monitor communications concerning scheduling, payment, and student progress.
- Submit payment through the provided card payment link by the agreed due date.
- Notify the Tutor of cancellations or scheduling changes at least 48 hours in advance.
- Remain available for emergency contact when appropriate
9. Student and Parent/Guardian Code of Conduct
To ensure a safe, respectful, and productive learning environment, all students and parents/guardians are expected to conduct themselves professionally throughout the tutoring relationship.
Student Conduct
The Student agrees to:
- Treat the Tutor with courtesy, honesty, and respect at all times.
- Arrive on time and prepared for each tutoring session with the necessary materials.
- Participate actively and make a reasonable effort to complete assigned work and practice activities.
- Follow all instructions provided by the Tutor during each session.
- Refrain from disruptive, disrespectful, abusive, threatening, or inappropriate behavior, including the use of offensive language or harassment.
- Use electronic devices only when they are required for tutoring purposes unless otherwise approved by the Tutor.
- Maintain academic integrity by completing their own schoolwork. The Tutor will provide guidance and instruction but will not complete assignments, quizzes, tests, or examinations on the Student's behalf.
Parent/Guardian Conduct
The Parent/Guardian agrees to:
- Treat the Tutor with courtesy, professionalism, and respect.
- Support the Student's attendance, punctuality, and participation in tutoring sessions.
- Communicate scheduling changes, concerns, or questions in a respectful and timely manner.
- Refrain from abusive, threatening, discriminatory, or harassing language or behavior toward the Tutor.
- Ensure that all payments are made according to the terms of this Agreement.
- Provide a safe, quiet, and distraction-free environment for online or in-person tutoring sessions whenever reasonably possible.
- Allow the Tutor to conduct each session without unnecessary interruptions, except when assistance is requested or required.
Violations of the Code of Conduct
Failure to comply with this Code of Conduct may result in a verbal or written warning, temporary suspension of tutoring services, or immediate termination of this Agreement at the Tutor's discretion.
Termination resulting from repeated misconduct, harassment, threatening behavior, or any violation of this Agreement shall not relieve the Student or Parent/Guardian of any financial obligations remaining under this Agreement.
10. Academic Integrity
The Tutor may explain concepts, review assignments, provide examples, and help the Student develop solutions. The Tutor will not complete graded assignments, examinations, or academic work on behalf of the Student.
The Student remains responsible for following all school, college, testing, and academic integrity requirements.
11. Academic Results
Tutoring is intended to improve the student's understanding, confidence, study habits, and academic skills.
The Tutor does not guarantee specific grades, test scores, promotions, admissions decisions, scholarships, or other academic outcomes. Results depend on several factors, including attendance, participation, preparation, and independent study.
12. Confidentiality
The Tutor will make reasonable efforts to protect the Student’s personal and academic information.
Information may be shared with the Parent or Guardian when the Student is under 18 or when permission has been provided. Information may also be disclosed when required by law or when necessary to address a safety concern.
13. Technology and Online Sessions
For online tutoring, the Student or Parent/Guardian is responsible for providing:
- A reliable internet connection.
- A working computer, tablet, or other suitable device.
- Access to the required video meeting platform.
- A quiet and appropriate learning space.
The Tutor is not responsible for delays or missed instructional time caused by the Student’s device, internet connection, or technical difficulties.
14. Termination of Services
Either party may terminate this Agreement by providing written notice.
If the Student or Parent/Guardian elects to terminate tutoring services before the end of the agreed contract term, the Student or Parent/Guardian acknowledges that they remain financially responsible for the remaining balance due under this Agreement. Any unpaid fees for the remainder of the contracted tutoring sessions or agreed payment plan shall become immediately due and payable unless otherwise agreed to in writing by the Tutor.
If the Tutor terminates this Agreement for reasons other than nonpayment, misconduct, or a violation of this Agreement, and a qualified substitute tutor is not provided, the Student or Parent/Guardian will not be responsible for payment for any tutoring sessions that have not yet been provided.
The Tutor reserves the right to terminate services immediately for nonpayment, repeated missed appointments, disruptive behavior, harassment, or violation of the terms of this Agreement. Any outstanding balance, including amounts due under the contracted tutoring term, shall remain the responsibility of the Student or Parent/Guardian.
